Schedule Your Appointment in a Few Easy Steps
On the computer
1. Click the “Schedule my Appointment Through Patient Portal” button
2. Login to your Patient Portal Account (view your health record)
3. If you have existing patient credentials, enter your username and password
4. If you cannot/do not remember your username and password, select “Using Mobile
Phone” option
5. Enter your First Name, Last Name, and Date of Birth to receive a one-time
password
6. In the Patient Portal, under “Appointments” select “New Appointment”
7. Select Kettman Family Practice or search for your provider
8. Schedule your in-person or telehealth appointment

On the Healow App
1. Log into your Healow App
2. Click on “Visits” or “Find Appointment”
3. Select Kettman Family Practice or search for your provider
4. Schedule your in-person or telehealth appointment
IMPORTANT:
You must enter a valid phone number to verify your appointment via text message or automated call
back.
Your appointment is not verified until you receive a text or automated call containing a verification code
and have entered that code on your booking form.
Be sure to keep your appointment booking page open in your browser until you have entered this code.

Bring your photo ID and primary/secondary insurance cards to your appointment to ensure your claim is processed properly.
